Costa Rica - S&R Please

My girlfriend and I booked a trip to Costa Rica. Any suggestions on places to stay, restaurants, fun adventures, etc.?

I've heard some points of interest include zip-lining, touring waterfalls, volcanoes, jungles, etc. Does anyone recommend a specific tour, fishing guide, etc?
